The purpose of this survey is to provide us with more information about your Trust's interest and readiness to be a site in the FAMOUS Trial. It is not an agreement to conduct the trial.  We are interested in your honest opinion.

We also ask that you please consult with colleagues and support departments before answering the questionnaire. 

The survey must be submitted online. However, to help you gather the information you need to complete this online survey, please consult a paper version of the questions which can be found by clicking here.
