Developing Good Practice Guidance for International Dementia Care in collaboration with the World Health Organization
Thank you for participating in this survey.
Following the launch of the "Guidance on community mental health services: Promoting person-centred and rights-based approaches" document by the World Health Organization in June 2021, they have been asked to create a similar document for dementia services. This is also in support of WHO’s global mandate - the Global action plan on the public health response to dementia 2017-2025 (WHA A70/28) - which envisions a world in which dementia is prevented and people with dementia and their caregivers live well and receive the care and support they need to fulfil their potential with dignity, respect, autonomy, and equality.
This survey will enable us to collect as many details about dementia services from around the world to help us create this document. We specifically would like to know the service contact details, the type of services they offer, and how they promote good practice in relation to human rights and person-centred approaches within these five areas: 1) respect for legal capacity; 2) non-coercive practices; 3) participation; 4) community inclusion, and; 5) person-centred approaches. The whole questionnaire can take a while to complete, so we have enabled you to finish the questionnaire later if you so wish. The data, however, will only be uploaded by clicking FINISH on the final page. From this point, it will not be possible to withdraw or change your service answers.

This is so we are able to follow up with you if we cannot get hold of the service you have submitted. Please note your details will not be disclosed or published, and will be stored separately from the service details. We will only be able to match up your contact details and your survey response using a passcode or memorable word you will be asked to provide below.
